REQUIREMENTS FOR MINORS (persons under the age of 18) |

- Completed online application.
- Acknowledgement slip.
- Payment slip.
- Copy of child’s birth certificate.
- Letter of consent jointly signed by child’s both parents [to be typed].
- Photocopies of the parent’s passport data pages. NOTE: At least one parent must be a Nigerian citizen.
- Additional documents could be required for single parents.
