- Passport Application Form to be completed online and printed out.
- Payment slip printout upon completion of Application form online.
- Acknowledgment slip print out.
- Photocopy of marriage certificate.
- Typed consent letter from spouse.
- Newspaper publication in the National dailies for marriages done in Nigeria only.
- Valid biodata page of husband’s E-passport.
- Evidence of payment of Administrative Charge (USD$60)
