- Passport Application Form to be completed online and printed out.
- Payment slip printout.
- Acknowledgement slip printout.
- Police Report (Translated in English)
- Submission of completed online E-passport Application Form.
- Two passport size photographs with white background.
- Applicants are to provide the correct passport number in the case of lost e-passport.
- Evidence of payment of Administrative Charge (USD$60)
